If you were a patient at a MedExpress center that has now closed, you can still obtain your medical records. MedExpress maintains these records in accordance with federal and state privacy regulations. This guide provides official information on how to request your records.
Official MedExpress Information
Patient care records are kept in strict confidence and in accordance with federal and state privacy regulations. The release of medical records requires a valid patient authorization or other processes, as required by law.
To obtain a copy of your medical records or information from it, please fill out and sign the Authorization for Release of Protected Health Information form. Then mail or fax it to the address provided below.

Fill Out the Form Completely
When filling out the form, be sure to:
- Complete all fields. If something doesn't apply, please note N/A.
- Clearly state who the patient is and the patient's date of birth.
- Clearly state who is to receive the records, even if the patient will receive their own records, and where the records are to be sent.
- Specify dates of service for which records are requested.
- Confirm that the patient or legal representative has signed and dated the form.
- If acting as the legal representative (unless a parent), submit the document that empowers you as legal representative (examples: Medical Power of Attorney, Appointment as Guardian or Conservator).
- As needed, specify the expiration date of the form, otherwise it will expire in 6 months.
NOTE:
Incomplete authorizations are not valid and cannot be processed. Please be sure to complete all fields of the authorization form.

Yes. Parents or legal guardians can request medical records for minors by completing the authorization form. Proper identification and proof of relationship may be required.
How can I request records for a deceased patient?
The personal representative of the deceased patient's estate can request medical records. Documentation showing your authority to act as the personal representative must be provided along with the authorization form.
